The Day of your Surgery/Procedure

What to do on the day of your surgery/procedure

Bring your insurance card, picture ID and any co-pay, co-insurance and/or deductible as discussed with you. Leave all other valuables at home, including jewelry.

Arrive promptly at the time scheduled. This will allow adequate time for all necessary admission procedures.

Wear loose-fitting clothing. You may take a shower the night before and/or the morning of your surgery/procedure with an antibacterial soap. Do not use any lotions, creams, powders, or make-up.

If you wear glasses or contact lenses, bring your eyewear case for their safekeeping during your surgery/procedure. If you have any removable dentures or bridgework, we will give you containers so those pieces can be safely stored during your surgery/procedure.

If you are a Parent/Legal Guardian for a Surgery Center patient who is under 18 years of age, you must accompany the minor on the day of the surgery/procedure. If you are the Legal Guardian you must bring proof of guardianship. Parent/Legal Guardian must remain at the facility until the minors discharge.

If you have an Advanced Directive or Power of Attorney for Health Care, please bring a copy with you on the day of your surgery/procedure.

Bring a list of your medications, including dosage and how many times a day you take them (You can complete the Medication Reconciliation form) Also bring a list of any allergies you may have.

If your child is having surgery/procedure, please feel free to bring his or her favorite comforting toy or blanket.
